Sign in to FAA MedXPress
Create an account if needed, or use your existing account. Complete Form 8500-8 carefully, review your answers and submit your application on the FAA website.
Save your confirmation number
The FAA provides a confirmation number after successful submission. Print or save the completed application and summary page. Dr. Berens needs this number to retrieve your application; creating an account alone does not complete this step.
Bring your number and identification
Bring your confirmation number and valid government-issued photo ID, such as a driver’s license or passport. The office also asks non-U.S. citizens to bring their passport.
Timing matters
Your examination must take place within 60 days after you submit your MedXPress application. Confirm an appointment date with the office before submitting too early.
What else to bring
- A copy of your completed MedXPress application.
- Current eyeglasses or contact lenses, if used.
- A list of medications and relevant medical records requested for your conditions.
- Any FAA Special Issuance Authorization or Statement of Demonstrated Ability (SODA), if applicable.
Ask the office about required records before your visit. Additional evaluation or FAA review may be necessary; an appointment does not guarantee issuance or same-day certification.
FAA examination services
The practice offers Class 1, Class 2 and Class 3 FAA medical examinations, pilot physicals and FAA consultations. Confirm the service you need when requesting your appointment.
