Privacy Policy

This notice explains information handling on the NuLook4U website. It does not replace the practice’s Notice of Privacy Practices for medical care.

Consultation requests

When you submit the consultation form, the website processes your name, email address, optional phone number, treatment selection, contact preference, message and acknowledgment to send your request to the practice through its email delivery provider, Resend. A phone number is required if you choose phone or text contact. The practice uses these details to respond to your inquiry.

Please provide only the information needed to arrange a consultation. Do not submit medical records, photographs, insurance details, diagnoses or a detailed health history. Ordinary email is not a secure medical-record portal and may not be encrypted end to end. Ask the office how to share clinical information appropriately.

The website application does not intentionally store form contents in a database or log them. The email provider and the practice’s mailbox process the message; hosting and delivery services may retain operational records under their own arrangements. Contact the practice with questions about access, retention or deletion.

Spam protection

The consultation form uses Cloudflare Turnstile to distinguish human visitors from automated submissions. Cloudflare processes technical browser information for this security check. The website sends the verification token, not your consultation fields, to Cloudflare for validation.

Website operation and maps

Hosting services process technical request information, including IP addresses, to deliver and protect the website. Embedded Google Maps connects to Google when a map loads and is subject to Google’s privacy practices. You can also use the direct Google Maps or Apple Maps directions links.

Optional analytics and your choices

Optional Google Analytics remains off unless you allow it through Privacy choices. It is limited to the homepage and location page on the main domain. Contact, treatment and confirmation pages do not load analytics. Advertising personalization is disabled in this implementation. Form contents, treatment selections and URL query values are not intentionally sent to analytics.

Your choice is stored in your browser for up to six months. Use Privacy choices to accept, reject or withdraw optional analytics. Global Privacy Control is treated as rejection. Browser storage may be unavailable or cleared; these choices do not affect access to care.
